The 1941's Atlantic Charter and Dumbarton Oaks proposals stressed over the application of right of self-determination, and later these charters and proposals turned the basis of United Nations charter. They don’t understand that the word â€Å"love† isn’t just a 4 letter word†¦ It’s way beyond that. This is what William Shakespeare is trying to clarify in his Sonnet 116. He wants to expound what love is, what it isn’t. Using a couple of metaphors, Shakespeare’s main aim is to elucidate the theme that real love is immortal, consistent and certainly not under the mercy of time. Shakespeare starts off sonnet 116 by saying that true love overcomes impediments and doesn’t get affected by the changes in the surrounding. Following that, he compares true love to a Lighthouse. How are the following related? Well, a lighthouse, as well all know, guides ships and at the same time, overcomes all of the storms tempests, at the same time, love as well guides couples and lost hearts to the right track and is able to overcome all the adversities it might encounter. Furthermore, Shakespeare continues with the metaphors saying â€Å"It is the star to every wandering bark†. Here, Shakespeare is comparing love to a star. Long time ago, people used to depend on â€Å"stars† as a source of guidance since compasses back then didn’t exist. Yet again, love is being compared to a star that leads the way for lost hearts. Lost hearts, blind eyes, love guides them all. In addition, Shakespeare stresses on the idea that love isn’t for time wasting or entertainment. Love isn’t restricted within a limited time, hence why Shakespeare describes what love is not: it is not susceptible to time. Priceless, consistent, immortal, permanent, this is what true love is all about in the eyes of Shakespeare. He definitely gives a clear description about what love is through the metaphors he included that contributed to his main clarification. Sonnet 116 revolves around a single theme, one that Shakespeare firmly believes in. In this cruel, twisted, tumultuous, and confusing world, there is only one constant force; Love. Shakespeare focuses on the idea that love isn’t temporary and does not fade with youth. Beauty disappears, youth fades; however, love endures till the afterlife. All the metaphors that Shakespeare included in this sonnet contributed to the theme that love is immortal. If one finds changes in the loved one, it must NOT affect love; otherwise, it wouldn’t be true love after all. What Shakespeare is basically saying is simply, love is constant, immortal, and consistent  and overcomes impediments. If it doesn’t fall under the following description, then it isn’t true love. To sum up, Shakespeare writes this poem in hope of clarifying the idea that love isn’t all about beauty, youth or price. It is priceless and isn’t under the clemency of time. The metaphors in sonnet 116 contribute in reaching a main theme, and that is what Shakespeare’s main aim is. It doesn’t matter how old you get, it doesn’t matter how rich or poor you are, it doesn’t matter how pretty or false you are because once you’re in love, all of these won’t matter anymore. I definitely agree on Shakespeare’s definition of true love for nowadays I believe it’s rare to find. With fingers crossed, I hope people would read this poem with a notepad in their hand to take notes and value love for its true meaning instead of overusing it.

I was eager to know what my friend's response would be. "Yeah," she replied, "I am. It's holy, respect for Jesus and Mary. Sometimes we have to do it as penance after confession." Inquisitively I asked, "I don't get it. So you perform this ritual for different reasons? What are you trying to accomplish when you do it, get into Heaven or just avoid going to Hell? Or could it be that it's just to do the same thing Catholics have always done?" Rituals, no matter how major or minor they seem, can be found almost everywhere. Some are of a religious context, and some are not. Some are performed for specific reasons, and some are performed simply to avoid change. The presence of rituals and their importance are very evident in the plays Everyman, written by an anonymous writer, and Endgame, written by Samuel Beckett. Everyman tries to prepare for death and attain his ultimate goal of gaining entrance into heaven by changing his life through the ritualized acts of scourging himself and performing the seven blessed sacraments. In Endgame, Clov ritualistically looks out of the window to make sure that nothing has changed and that death, or anything else that may disrupt the character's repetitive cycle of life, is not on the "horizon." Hamm also resists change and attempts to avoid death by having Clov continually make sure that his chair is in the proper location so that death cannot find him in the wrong place and sneak up on h im. The rituals are very different, and the major contrast between those performed is that Everyman realizes that the coming of death is inevitable and he wants to do whatever necessary to prepare for it, but the characters in Endgame fear death, and rather than prepare for it, try to avoid it by resisting any change to their daily routines. Everyman does not resist death and even prepares for it by performing the religious rituals of the seven blessed sacraments and scourging himself. Through the performance of rituals Everyman is trying to attain the ultimate goal of reaching Heaven. He finds that the only character that will accompany him on his journey is Good Deeds, but she is weak. This represents the idea that he has not done enough good during his life and must now do something to change.

I'm doing some initial stretching in the basement, when my father comes down for the first time since I have been home. â€Å"Pat?† he says. I stop stretching, stand up, and face him. He's on the last step, stopped as if he is afraid to set a foot down on my territory. â€Å"Dad?† â€Å"You certainly got a lot of equipment down here.† I don't say anything, because I know he is probably mad at my mother for buying me a gym. â€Å"There's pretty good Eagles coverage in the papers today,† he says, and then extends the sports sections of the Courier-Post and The Philadelphia Inquirer to me. â€Å"I got up early and finished reading both so that you could keep up with the team. By your comments yesterday during the game, I could tell you don't know all of the players, and I thought maybe you'd like to follow along this season now that you're home and – well, I'll just leave them on the top step from now on.† I'm too shocked to speak or move, because my father has taken the sports pages with him to work ever since Jake and I were little kids. As my stomach muscles crunch and burn, I worry that my father is only playing a mean trick on me and that the papers will be the entertainment or food sections, but when I finish the sit-ups and make my way to the steps, I see that Dad really did leave me the sports sections of both papers. When it is time for me to take my a.m. pills, I find my mom in the kitchen cooking eggs. My plate is set at the breakfast bar, and my five morning pills are laid out in a line on a napkin. â€Å"Look,† I say, and hold up what my father gave me. â€Å"Sports pages, eh?† Mom says over the sound of frying eggs. â€Å"Yeah.† I sit down and pop all five pills into my mouth, trying to decide how many I will swallow today. â€Å"But why?† Mom scrapes the eggs from the pan and onto my plate with her spatula. She smiles and says, â€Å"Your father is trying, Pat. But I wouldn't ask too many questions if I were you. Take what he gives you and be happy – that's what we do, right?† She smiles at me hopefully, and right then I decide to swallow all five pills, so I take a sip of water and do just that. The big news is the upcoming Giants game, which everyone thinks will be the key to winning the NFC East, especially since the Giants have already lost to the Indianapolis Colts in game one. A loss will put them at 0 – 2 and the Eagles at 2 – 0. The game is being hyped as a big one, and I have a ticket, thanks to Jake, which makes me really excited. Each night, I wait for my dad to come home from work, hoping he might want to talk about the upcoming game with me – so I can use the current players' names and prove to him that I am a real fan again – but he always takes his dinner into his study and locks the door. A few times I actually go to his study and raise a fist to knock, but I chicken out every night. Mom says, â€Å"Give him time.† Sitting in the brown recliner, I talk about my dad with Dr. Cliff during my Friday appointment. I tell him how Dad is leaving me the sports sections now, and how I know this is a huge deal for Dad, but I wish he would talk to me more. Cliff listens, but says little about my father. Instead he keeps bringing up Tiffany, which is sort of annoying because she has only been following me when I run, and that's about it. â€Å"Your mother says you are going to the beach with Tiffany tomorrow,† Cliff says, and then smiles like men sometimes do when they are talking about women and sex. â€Å"I'm going with Ronnie and Veronica and baby Emily too. We talk a little more about how I made it through a whole week without having an episode, which is evidence that the drugs are working, according to Cliff – because he doesn't know I spit at least half of the pills into the toilet – and when it is time for me to go, Cliff says, â€Å"I just have one more thing to say to you.† â€Å"What?† He shocks me by jumping to his feet, throwing both hands in the air, and yelling â€Å"Ahhhhhhhhhhhhhh!† So I jump to my feet, throw both hands in the air, and yell â€Å"Ahhhhhhhhhhh!† too. â€Å"E!-A!-G!-L!-E!-S! EAGLES!† we chant in unison, spelling the letters with our arms and legs, and suddenly I am so happy. Cliff predicts a 21 – 14 Eagles victory as he walks me out of his office, and after I agree with his prognosis, we enter the waiting room and my mother says, â€Å"Were you two just doing the Eagles chant?† Cliff raises his eyebrows and shrugs his shoulders at my mother, but when he turns to walk back into his office, he begins whistling â€Å"Fly, Eagles, Fly,† at which point I know that I am seeing the best therapist in the entire world. On the drive home, my mother asks me if Cliff and I talked about anything other than Eagles football during the therapy session, and instead of answering her question, I say, â€Å"Do you think that Dad will start talking to me at night if the Eagles beat the Giants?† Mom frowns, grips the steering wheel a little harder. â€Å"The sad reality is he might, Pat. Ibuprofen and Acetaminophen Essay

Organic chemical compounds contain a vast number of isomers, molecules with the same molecular formula but different atomic arrangement, of which there are three types: structural, geometric and optical. The most closely-linked of isomers are optical isomers, which differ only by the three-dimensional placement of the molecule’s attachments, which renders its mirror images to be non-superimposable. The presence of optical isomerism within a molecule is determined by the existence of a chiral centre—a carbon atom with four different groups (see Appendix, figure 1). Pure optical isomers have identical physical properties such as melting point, boiling point and density, as well as identical chemical properties; thus there are only two ways that they can be distinguished: their interaction with other chiral substances and their interaction with plane polarized light. An enantiomer (one out of a pair of optical isomers) rotates plane polarized light in the opposite direction of which the other enantiomer rotates the plane polarized light, of equal magnitude (see Appendix A, figure 2). An enantiomer that shifts plane polarized light to the left is given the prefix â€Å"L† (levorotatory) or â€Å"S†, while an enantiomer that shifts plane polarized light to the right is given the prefix â€Å"D† (dextrorotatory) or â€Å"R†. Despite their physical and chemical similarities, optical isomers are known to have substantially different behaviours within the human body. This is due to the fact that enzymes and receptors in the body are stereospecific, meaning that they can interact with one enantiomer of certain molecules and not the other. For instance, the human body can only break down D-glucose (dextrose) for energy but not L-glucose, and can only utilize L-amino acids rather than D-amino acids. While one form of a stereoisomer may be beneficial, the other may be ineffective or even harmful, in some cases. Thus, it is crucial that optical isomerism is taken into account during medicinal drug development and usage. For example, the sedative thalidomide was available in Europe in the 1960s for purposes of alleviating morning sickness in pregnant women. However, the drug was sold as a racemic mixture, which contains equal portions of both enantiomers (this would not shift plane polarized light as the two enantiomers shift in different directions),  and while R-thalidomide (see Appendix A, figure 3) works effectively as a sedative; S-thalidomide can cause genetic damage leading to mutation of the fetus. Consequently, 12 000 infants were born worldwide with malformation of the limbs. To further illustrate the importance of optical isomers in drug action, the drug ibuprofen can be explored. Ibuprofen is a drug used for anti-inflammatory purposes such as pain relief, fever and swelling reduction, and is classified as a nonsteroidal anti-inflammatory drug (NSAID). Derived from propanoic acid in the 1960s by a pharmacy chain called Boot’s UK Limited, ibuprofen was initially launched as a treatment for rheumatoid arthritis, and was awarded the Queen’s Award for Technical Achievement in 1987. It is currently available under a variety of trademarks such as Advil, Motrin, Nurofen, and Brufen, among others. Ibuprofen works by inhibiting the enzymatic action of cyclooxygenase (COX1 and COX2) within the body, which catalyzes the conversion of a compound called arachidonic acid into prostaglandins. Prostaglandins are locally-acting hormones that cause swelling, heat, loss of function, fever and pain, collectively known as inflammation, at a site of injury through the accumulation of white blood cells. Through inhibiting this reaction, painful symptoms can be reduced or eliminated. The IUPAC name for ibuprofen is 2-(4-(2-methylpropyl) phenyl) propanoic acid. It is a carboxylic acid which also contains a phenyl group (see Appendix A, figure 5). Due to its chirality, ibuprofen has two enantiomers (see Appendix A, figure 6): S-ibuprofen, which rotates plane polarized light to the left and is pharmacologically active, and R-ibuprofen, which rotates plane polarized light to the right and has no anti-inflammatory effect (and is thus inactive as a drug, since . However, ibuprofen is sold on the market as a racemic mixture. Thus, a dose of ibuprofen contains only 50% of the active enantiomer, S-ibuprofen. Nevertheless, R-ibuprofen undergoes species-specific chiral inversion within the body, in which approximately 60% of R-ibuprofen is converted into S-ibuprofen. The mechanism of the inversion is through an enzyme, alpha-methylacyl-CoA racemase (AMACR) that is present in the liver, the kidney and gastrointestinal tract. A  substitution nucleophilic bimolecular (SN2) r eaction must take place (see Appendix B, figure 1), which results in the complete conversion of every molecule of one enantiomer to the other. In this case, only R-ibuprofen is converted into S-ibuprofen (see Appendix B, figure 2). The benefits associated with ibuprofen use include its efficiency as a NSAID in decreasing inflammation, in addition to be non-addictive and affordable. However, ibuprofen often causes stomach irritation, and can impede concentration and cause drowsiness. It may also result in a variety of adverse side effects (see Appendix C, figure 1). An alternative to ibuprofen is the drug acetaminophen, also known as paracetamol. Acetaminophen, (see Appendix A, figure 7), is commercially available under brand names such as Tylenol and Anacin (see Appendix A, figure 8). It works by inhibiting the synthesis of prostaglandins. It does not, however, have any anti-inflammatory action and thus only targets tissue of the nervous system—easing pain without being directed towards the root of the problem. Acetaminophen is known to cause fewer side effects than ibuprofen; however for a list of side effects, see Appendix C, figure 2. Acetaminophen does not have a chiral centre, and therefore does not have any optical isomers. Both drugs are used to relieve headache pains and fever; however their varying properties impact their function and effectiveness in treating specific symptoms. The solubility of ibuprofen in water is 0.0002M. Although its carboxyl functional group is highly polar, the large non-polar component greatly decreases its overall polarity. As â€Å"like dissolves like†, ibuprofen does not dissolve well in water, which is polar. To solve this problem, ibuprofen is reacted with the amino acid lysine (see Appendix A, figure 9), which together forms the salt ibuprofen lysinate. Ibuprofen is then released into the bloodstream in which the reaction can reverse. On the other hand, acetaminophen is highly soluble in water; with a solubility of 0.091M. Because it contains an amide and an alcohol, there is a substantial degree of hydrogen bonding resulting in high polarity (see Appendix A, figure 10). This allows the drug to be absorbed into the bloodstream effectively. Ibuprofen has a pKa (acid dissociation constant) of approximately 4.43, while the pKa of acetaminophen is 9.51 and 25 °C, making ibuprofen more acidic. Because neutral substances pass through bodily membranes more easily, acetaminophen is more easily absorbed. Acetaminophen also has more acid stability. Administered orally, an ibuprofen capsule has an enteric coating, which prevents stomach acid from breaking down the drug before it reaches the small intestine, where it is absorbed. A film coating is used on a tablet of acetaminophen, which protects the tongue from the contents, as well as protecting the contents from moisture and light. Within the body, the film can be broken down by saliva or stomach acid, and the way in which the drug is absorbed is not affected. The Montagues and the Capulets start to fight due to the provocation. After Mercutio died, Romeo kills Tybalt as the revenge. Adolescents advocate violence rather than peace to solve the conflict. It is an immature way to solve the conflict and argument by adolescents. Violence is an issue among adolescents. They ignore consequences, social rules and laws to fight in the public space (Cox, 1967). The consequence is both Mercutio and Tybalt died in the fighting. Romeo is banished from Verona because he disobeys the law. Characters in the play pay the price for their impulsive fighting. Their impulsive behaviours lead to more tragedies. Another feature for adolescents is love. Adolescents are curious about love and sex. Boys and girls are easily attracted to each other. Romeo and Juliet fall in love at first sight. Romeo forgets Rosaline immediately. Juliet is also a young girl at the age that expects love. They fall in love impulsively and get married secretly in a very short time, in which is hard to know each other well. Adolescents are at the age that wants to be independent and have the desire for the opposite sex. They want to be independent of their families and parents. They spend more times with peers than parents (Cox, 1967). The power of love among two young adolescents makes Juliet refuse the arranged marriage and commit suicide due to the immature love. Adolescents are rebellious, and they are less immature than adults. Therefore, parents play important roles to guide and protect their children. Both Romeo and Juliet are a lack of parenthood that has weak relationships with their parents. They do not get support and understandings from their families (Cox, 1967). When Romeo feels painful towards Rosaline, he would rather find his friends and Friar Laurence rather than his father. He asks for help from Friar Laurence to hold a wedding. Friar Laurence is more like Romeos father, and he understands Romeos emotions. Friar Laurence is a reliable person for Romeo when he feels desperate. Montague knows nothing about Juliet until the end of the play. There is a gap between Montague and Romeo. There are not too many scenes about Lady Capulet as well. Lady Capulet follows her husband to prepare the wedding for Juliet and Paris regardless of Juliets intentions. When Juliet shows her attitudes towards the marriage, Lady Capulet ignores Juliet and refuses to talk to her. Juliet would rather share her secrets with the Nurse rather than Lady Capulet. As Cox argued, Juliet is not close to her mother. Juliet does not feel the love from her mother when she is desperate. Lady Capulet is absent when Juliet needs her. She ignores Juliets emotions towards the marriage no matter how Juliet is begging. The unconcern from Lady Capulet and cold mother-daughter relationship leads the suicide. It is obvious that Romeo is closer to Friar Lawrence and Juliet is closer to the Nurse than their biological parents. Adolescents are sensitive, in which they need more concerns from their parents. Parents should protect their children falling into a wrong path. Montague and Capulet are regretful for R omeo and Juliet. If they could concern more about childrens emotions during the adolescence instead of feud and power, perhaps death would not happen. Marjorie Kolb Cox analysed Romeo and Juliet from a psychological aspect. Romeo and Juliet are at adolescence period. The audience is convinced why some impulsive behaviours occur, why there are many fighting scenes in the play and why Romeo and Juliet fall in love so quickly. The death of Romeo and Juliet, as well as other characters, is a consequence of the effect from adolescence. There is no careful consideration for adolescents, and they are not aware of the responsibility. Cox makes readers focus on reasons behind the tragedy instead of a love story. In this day and age, many argue that the United States has an â€Å"apathy problem.† According to The Huffington Post, â€Å"America is truly exceptional in its ability not to care.† The only time we are concerned about an issue, is only if and when it personally affects us. This is also the circumstance in regards to politics. Citizens must have reliable information in order to make good, rational decisions that will help our political system run efficiently. Unfortunately, this is not always the case. According to Converse, â€Å"most Americans glance at public life innocent of ideological concepts and unguided by any broad outlook on government policy† (Kinder and Sanders, 1996, p. 167). In such a society, there is no such thing as equal representation, only popular representation, as a result of the disparities and ignorance of the people. Converse makes very valid arguments that confirm that regardless of ideological innocence, political electorates, as a wh ole, will continue to be consistent with the changing conditions of society. In the past, the â€Å"information problem† was the scarcity of information and the disconnected flow of information to the voters. Today, the problem is one of â€Å"superabundance† (Converse, 1990, p. 371). Because there is such a massive volume of information available, it is difficult to organize and rummage through it all. Corresponding to Down’s Rational Choice theory, rational voters will not make an effort to analyze such information because there are higher costs than benefits to search through and find â€Å"good† information (Converse, 1964, p. 33). Consequently, there is a huge disparity—a difference that we greatly underestimate—between the ignorant and the well-informed constituents. According to Converse, there... ...presentation fidelity â€Å"is higher for the better-educated constituents† (Converse, 1990, p. 387). This is the case because politically sophisticated entities will be more vocal and more involved in legislation. Our society is a mix of the two models, issue-public and top-down, so it only makes sense that information will be unequally distributed. Although we don't like to admit it much, we are a very indifferent society, so people do not care; they will simply delve into and further develop their expertise on topics that they are specifically interested in. However, there is no need to fret. Everyone is different and has distinct interests and skills. After all, there are nine different types of intelligences—some excel in Math while others outshine in English. Therefore, we should leave politics to the professionals and the well-informed constituents.

This essay will critically analyse my performance throughout the Objective structured clinical examination (OSCE) assessment I completed, including the escalation strategy utilised by the Nation early warning scores (NEWS) (RCP, 2012) as a track and trigger tool (NICE, 2007). Based on the findings from the assessment interventions will be recommended and supported by evidence and formatted on the ABCDE approach I used in the OSCE. The ABCDE assessment is used as a tool to assess for the critically ill patients Airway, breathing, circulation, disability & elimination. (RCUK, 2005). It is a systematic approach that can assess the severity of the critically ill patient, assess and treat life threatening conditions and have rapid intervention when needed (Grindrod, 2012). During the Assessment I introduced myself to Mrs Jones to remained respectful, non-discrimitive and ensuring the comfort and dignity of my patient, to which I pulled the curtains (NMC, 2008). I gained verbal consent from the patient to carry out the physical assessment (NMC, 2008), although I should have gained consent at the beginning when I started talking to the patient. This is important because the patient needs to understand the proposed assessment, according to the NMC (2008) the process of establishing consent should demonstrate a clear level of accountability. If consent is refused then the patient’s wishes should be respected although the patient needs to be fully informed of what can happen (NMC, 2008). Standard precautions are put into place in the clinical setting to protect patients and staff which are vulnerable to infection. Alcohol based hand rubs are at the point of contact of each patient (NPSA, 2008) to help prevent hospital acquired infections and cross contamination (DOH, 2009), which I used prior to seeing Mrs Jones. Airway The assessment of Mrs Jones airway went well I assessed for an open airway by alking to her to see if there was any vocal response, Mrs Jones responded coherently so there was a patent airway, no noises were heard which can indicate partial obstruction of the airway (RCUK, 2010). Mrs Jones was able to cough to clear secretions independently. Lack of oxygen can lead to anaerobic respiration at a cellular level which produces acidosis as lactate is produced which can lead to hypoxia (Jevon, 2011). Breathing I looked for evidence of hypoxaemia by assessing mouth and oral mucosa for central cyanosis (O’Driscoll et al, 2008), none was evident. Respiratory rate was assessed over 1 full minute to ensure accuracy (Hunter, 2008) as deviation of 4 or more can be clinically significant (Subbe, 2006) The rate was raised at 24 which I record on the observation chart and the resperation rate falls in the orange band generating a score of 2, The acceptable normal respiration rate is 14 – 18 breaths per minute (Mallett & Doherty, 2001) indicating Mrs Jones could be compensating for metabolic alkalosis and It also contributes to the diagnosis and management of a variety of pathological conditions and helps to evaluate therapeutic interventions. Monitoring the patient’s respiration level is one of the most accurate indicators of deterioration, which is often poorly monitored and recorded Cretikos (2008). Accessory muscles should have been observed to assess for increased work of breathing, which would result in inadequate ventilation and poor gas exchange (Esmond, 2003). Oxygen saturations are considered the fifth vital sign (BTS,2008), and these were reduced at 93 %, normal range is 94% to 98% (BTS, 2008). I record on the observation chart whichs generates a score of 2. The drug chart was checked to see if target saturation has been identified and oxygen prescribes as per BTS (2008) guidance, and so 2L of oxygen was give via a nasal cannula to increase saturations to within target range. Mrs Jones was also sat up to increased functional residual capacity which helps to reduces the work of breathing helping to improve oxygenation (Kennedy, 2007). As per BTS (2008) guidance saturations were checked after 5 minutes and had risen to within target range. Crackles were heard on inspiration when I listened to Mrs Jones chest, this can be an indicator for pulmonary Oedema or pneumonia (Sheppard, 2003). Circulation Mrs Jones looked unsettled and felt cool and clammy, her radial pulse was easy to palpate but was very irregular which made me instigate an ECG, manually Mrs Jones pulse was 85bpm but recorded on the ECG was 114bpm that showed evidence of atrial fibulation (AF), the patient didn’t have a history of AF. Capillary refill was just over two seconds and blood pressure was115/85, I did not calculate the pulse pressure or arterial pressure. Her temperature was within normal range at 36. 3. I record the observations and the heart rate falls in orange band generating an additional score of 1. Mrs Jones explained that she had passed urine 5 hours ago which was 200mls. NICE (2007) state that an adult urine output should be measured at ‘>0. 5mls/kg/hr’, I knew this was low for the patient but I did not use the calculation to work out how much it was an hour, volumes of less than 0. 5ml/kg/hr can indicate cardiovascular compromise and renal impairment can occur (Dutton, 2012). Mrs Jones has signs of ankle oedema, which made me concerned for her fluid status so a fluid chart was commenced of intake and output. Disability Mrs Jones was awake and responding to myself using the AVPU tool, The AVPU scale is a quick and easy method to assess level of consciousness which can be affected by hypoxemia and hypercapnia (Palmer et al, 2006). It is ideal in the initial rapid ABCDE assessment (Smith, 2003) although a full assessment would require using the Glasgow coma scale (NICE, 2007). Mrs Jones blood glucose level was checked as this can rise as a result of sympathic activation, but the level is within normal range. Exposure With Mrs Jones consent I checked her invasive lines for phlebitis and her skin for any rashes, erythema or signs of pressure sores, all were normal and no phlebitis was noted. I did not assess to see if Mrs Jones had sacral Oedema, oedema only becomes apparent when the interstitial volumes has increased by 2. 5 – 3L (Porth, 2007) possible caused by heart failure. Care Escalation I documented all the patients’ observations on a NEWS Chart which generated a score of 7, this score then gives me appropriate actions to take as there is a marked deterioration of the patient. 7 or more triggers the Action of escalating care by contacting the medical registrar looking after the patient and also consider moving the patient to a level 2 or 3 care facility. When contacting the registrar I used the Situation, background, assessment and recommendation (SBAR) briefing model to tell the medical registrar about the patient so they are fully aware of the patient and their condition and actions I want them to take. The handover I gave to the registrar was slightly muddled and I jumped back and forth instead of remembering the systematic order that the tool was designed for there for I missed out information about Mrs Jones that could of been highly important to the doctor. Conclusion The ABCDE assessment gives health care professionals a framework which helps detect life threatening conditions and are addressed early. The patient I had during my assessment had a lot of complex issues but This approach helps remember the essential things and intervening and referring along the continuum of A to E helped reduce further determination progressing.
